VOLUNTEER AGREEMENT / RELEASE AND WAIVER FORM
In applying to perform duties for the Waterloo Busker Carnival as a volunteer, I fully understand and agree to the following:
1. That I will not be participating in volunteer activities in the capacity of a Waterloo Busker Carnival employee or independent contractor.
2. That I will not receive any remuneration, salary, wage or payment whatsoever, or be covered by the Workplace Safety and Insurance Act, 1997, S.O. 1997, Chapter 16, Schedule A.
3. That except as authorized, I will not divulge or make any use of confidential information.
4. That I will immediately notify the appropriate Waterloo Busker Carnival Executive Committee member of any incident that involves personal injury or property damage during my volunteer duties.
5. That either the Waterloo Busker Carnival Executive Committee member or I myself may terminate my volunteer activities at any time.
6. I acknowledge that my volunteer activity may involve personal risk of damage or injury. Notwithstanding this acknowledgement, I hereby release the Carnival, its Directors, Volunteers and agents from all claims for damage or injury to myself resulting from my participation as a volunteer.
7. I understand that information provided in my application to volunteer is entered in password-protected web-based volunteer management software that is administered by the Carnival's Volunteer Directors.

A MINOR PERSON WHO IS VOLUNTEERING MUST HAVE THEIR PARENT OR LEGAL GUARDIAN COMPLETE THEIR WAIVER BEFORE THE PERSON’S FIRST SHIFT AT THE CARNIVAL. AN EMAIL WILL BE SENT TO THE PARENT OR GUARDIAN WITH DIRECTIONS ON HOW TO SIGN OFF ON THE WAIVER.

THE PARENT OR LEGAL GUARDIAN OF THE MINOR PERSON VOLUNTEERING must also review the information under ‘VOLUNTEER AGREEMENT’ and agree to the following:
I hereby certify that I am the parent or legal guardian of the above named applicant, a minor pursuant to the Age of Majority and Accountability Act, R.S.O. 1990, c. A.7, and that she or he has my permission to serve as a volunteer with the Waterloo Busker Carnival. As the parent or legal guardian, I fully understand and have full knowledge of the nature and extent of the risks involved with his or her participation as a volunteer (see below).
